Coordinates and monitors the assessment, goal process and follow through for participants. Serves as an advocate in promoting job-readiness by uplifting, educating, and empowering participants to obtain and maintain employment.

This position will be opened until filled.

Coordinates and monitors the assessment, goal process and follow through for participants. Serves as an advocate in promoting job-readiness by uplifting, educating, and empowering participants to obtain and maintain employment.

Outreach and recruitment	?Increase awareness of the SCSEP in assigned counties. Recruits participants according to DOL guidelines
?Case Management	?Provides case management of participants including: determines participant eligibility; assigns participants to appropriate community service assignments; develops the Individual Employment Plan; provides regular monitoring of host agencies and participants; places participants into unsubsidized employment; and follow ups on retention. Implement a Participant Advisory Council Hold mandatory participant meetings
?Job Development	?Provides training on job retention and job readiness by holding regular job clubs and providing individualized job coaching. Develop relationships with local employers Develop a local Business Advisory Council
Data collection and documentation	?Completes all required paperwork and data collection requirements including; completing/recording participant enrollment and ongoing paperwork and eligibility documentation; keeps detailed case notes on all participants; completes required exit and follow up documentation; and delivers employment customer satisfaction surveys
Program Knowledge	Learn and maintain program knowledge by utilizing available resources and attending all mandatory trainings and staff meetings.
Outcomes	Encourage, promote, and provide?guidance to participants to obtain employment.

Essential Job Requirements:
Education	Degree in social services or related field preferred but not required.
Experience	Two years experience completing assessment and case management duties. Proven experience working with persons with diverse backgrounds.
Required Skills	Position proven oral, written, organizational, and record keeping skills. Knowledge of local community resources. Ability to move from one task to another quickly and with ease. Ability to manage change with a positive attitude. Posses the ability to motivate participants and encourage their self-efficiency.
Physical Requirements	Ability to move independently or with reasonable accommodation within the facility and community. Applicant must be physically and mentally able to perform all requirements of the job.
Background Check	Applicant must pass a background check.
